Executive summary

Purpose of Intervention

This intervention at the Devonport Royal Dockyard Limited (DRDL) licensed site was undertaken as part of the 2019/20 intervention plan and propulsion sub-division strategy.

Interventions Carried Out by ONR

I conducted a Licence Condition (LC) 07 (“Incidents on the site”) compliance inspection. I focused on the implementation of the licensee’s arrangements. This involved examination of relevant licensee documentation and their incident management system, and targeted discussions with relevant members of the licensee’s staff.

I conducted a LC 07 ‘Incidents on the site’, compliance inspection on the implementation of the licensee’s arrangements, which included a sample of the management system arrangements.

The intervention was performed in line with ONR's guidance requirements (as described in our technical inspection guides) in the areas inspected.

I utilised the following guidance during this inspection:

  • ONR Nuclear Safety Technical Inspection Guides (TIG’s) 07, NS-TAST-GD-007 Rev. 5, LC7 compliance.
  • NS-G-2.11: A system for the feedback of experience from events in nuclear installations (IAEA Guidance).

The intervention involved discussions with DRDL staff and examination of the extant arrangements and their implementation.

Explanation of Judgement if Safety System Not Judged to be Adequate

Not applicable.

Key Findings, Inspector's Opinions and Reasons for Judgements Made

The licensee’s staff engaged positively during the course of this inspection. During this inspection I have identified good practices and noted observations for management action.

The licensee is planning to improve their existing safety management arrangements for LC7 to align with relevant good practice. I found specific areas for improvement in relation to the implementation of the LC7 arrangements and event ownership responsibilities, action management and the quality of investigations. These shortfalls will be addressed through the raising of a regulatory issue and extant regulatory issues.

Conclusion of Intervention

Based on the evidence sampled during this inspection, I have concluded that an intervention rating of AMBER is appropriate (seek improvement).
