Confirmation that corrective actions from Springfields Fuels Ltd investigations into events on site are being adequately progressed, monitored, closed and that appropriate learning from the events is secured to prevent re-occurrence.

Key findings, inspector's opinions and reasons for judgement made
The inspection at Springfields Fuels Limited focused on corrective actions identified from investigations undertaken on incidents reported to ONR under licence condition (LC)7 incidents on the site.
The inspection sampled corrective action management and close out information. It provided confidence that corrective actions from investigation of incidents are well managed proportionately to the incident safety significance. I was content that the actions completed following investigations would ensure risks to workers would be minimised and help prevent a recurrence.
Opportunities for improvement of the corrective action process were identified from the inspection process but I consider these as refinement of the process rather than systemic shortfalls.
Corrective actions for security related incidents are managed by a slightly different process due to the security limitations of the dutyholders corrective action database. These differences have not been documented within the dutyholders business management system. A level 4 regulatory issue has therefore been raised to for the dutyholder to formally record the full process for the management and governance of security related corrective actions.
Conclusion
The inspection focused on corrective action management. Based on the sampling undertaken, I rate the LC7 and LC17 intervention as GREEN – ‘No Formal Action’. This rating is in line with the established ONR guidance.

None of the findings in the inspection presented an immediate or direct risk to nuclear safety.
